I have been presented a copy of Sauk Valley Community College's Acceptable Use of Technology and am aware a network code of Conduct exists. By initialing this form I agree to abide by the College policy and regulations on the appropriate use of technology resources. I understand that any violation of the terms and conditions contained in the Acceptable Use Policy may result in disciplinary or legal action by the College, Federal, and/or State officials. No user shall have an expectation of privacy in his/her use of Computer technology, including e-mail messages and stored files, except proprietary research by faculty members who need to protect work, product, etc.

Sauk Valley Community College is a Public Community College. The confidential material of our Board, students, and employees must be guarded with integrity and respect. Any breach of trust or breach of confidential material and information could lead to personal, professional and financial loss and hardship for our students and staff; and this further could lead to personal and professional naming of any employees of "Sauk Valley Community College" as well as "Sauk Valley Community College" in a lawsuit. The undersigned employee of "Sauk Valley Community College" understands and agrees that the legal, personal, and professional material handled by the College should be kept to the utmost confidentiality and free from release to any individual not authorized to access such information. I,therefore, agree to abide by the Confidentiality Notice and Agreement to keep confidential and privileged material known by me to remain known exclusively unto me and the employee of Sauk Valley Community College.

Information about individual students is collected and maintained with the understanding that it is used only as needed to support Sauk Valley Community College's basic educational requirements. Its use is restricted by College policy and by the Family Educational Rights and Privacy Act of 1974 (as amended). Access to most student data is limited to those people within the college who have a legitimate academic interest and a need to review an education record in order to fulfill their professional responsibility. This interest is defined as the demonstrated need to know by those who act in the student's educational interest, including all employees.I understand that by virtue of employment at Sauk Valley Community College, I may have access to records which contain individually identifiable information, the disclosure of which is prohibited by the Family Educational Rights and Privacy Act of 1974 (as amended). FERPA I will maintain the data to which I have access in strictest confidence. The information viewed will not be shared in any manner with others who are unauthorized to view such data. I understand that unauthorized copying of student files or verbally relaying any information contained in a student's file to another individual who does not have a legitimate academic interest may result in disciplinary action, loss of employment, and/or possible sanctions. I have read and understand and agree to abide by the FERPA guidelines.
